For all samples send to AMS Laboratories for analysis or testing, please include a completed copy of the Sample Submission Form.  The sample submission form can download from  following format .For help in completing the Sample Submission Form, please contact General Enquiry at (02) 9704 2300 or info@amslabs.com.au.

When sending samples to the lab for analysis:

1. Download & complete Sample Submission Form (Please include submission form in or on the box of samples). 

2. Ship samples and submission form to:        

AMS Laboratories Pty Ltd

8 Rachael Close, SILVERWATER, NSW  2128

3. Incoming samples are received by the Office Administrator who opens the packages, check the integrity of the samples and that the number of samples received correlates with the Sample Submission Form.
